Step 1: Please fill out our membership application form below.

Step 2: You may submit your membership contribution payment with a credit card by filling out the form below. Or you can submit your payment by check--made payable to Northwest Village Network--to P.O. Box 4330, 8227 Germantown Avenue, Philadelphia, PA 19118.

Step 3: Upon receipt of payment, you will receive a welcome email and also instructions about how to create a member login for the Member Portal on our website. You will also receive a welcome letter and membership card(s) in the mail.

Reduced Rate Individual - Available for those in need and not able to pay full cost of membership: $50.00 for 12 months

Reduced Rate Household - Available for those in need and not able to pay full cost of membership: $75.00 for 12 months

Individual: $125.00 for 12 months

Household: $175.00 for 12 months
